New Patient Registration

Catchment Area

To register with Sobell Medical Centre you will need to fill in the necessary registration forms available on request from the receptionist or you can print off the registration form , fill it out and bring it to the reception. you also need to complete the New Patient Health Questionnaire forms.

Please provide your NHS number and details of your previous GP. We need your NHS number for the electronic transfer of your medical records and accuracy of your registration details.

If you are registering for the first time in this country you will not have an NHS number

Once you have completed the registrations forms you will need to book and attend for an appointment with the Practice Nurse or the Health Care Assistant for a New Patient Health Check

Patients will not be unreasonably refused registration based on:

  • Medical condition
  • Race
  • Gender or sexual orientation
  • Disability
  • Age
  • Religious group or religious beliefs
  • Political beliefs
  • Appearance or lifestyle.

The Practice will however refuse registration if:

  • The list is officially “Closed”
  • The patient resides out of the publicised Practice area (please enter your Postcode at the top of the page)
  • The patient has been previously removed from the list;
  • The patient has a known history of violence.

The reason for refusal will be in writing and recorded in a permanent record for that purpose. This excludes Temporary Residents, where no record is necessary.
